Prostate enlargement : The prostate is a reproductive gland the size and shape of a chestnut, located below the bladder at the junction of the urinary and reproductive tract. The prostate secretes nutrients and fluids that facilitate sperm movement. The weight of the prostate increases between birth and adulthood and stabilizes in adulthood at 15 to 20 grams. Men from the age of forty may experience their enlargement so that they continue to grow. In certain cases, it may be up to 7 times its original size

What is benign prostatitis

BPH is an enlargement and swelling of the prostate that causes pressure on the urethra.

What are the symptoms / effects of the different benign prostatitis on an individual’s health ..?

BPH can lead to various problems,

  • such as frequent urination or a low and intermittent flow of urine.
  • Urinating every short period
  • Pain during urination.
  • Relationship problems, which in many cases may plague the marital relationship, includes : problems with erection, problems with sexual intercourse, dissatisfaction, feeling something that is missing in the sexual relationship
  • and finally the lack of lust and sexual desire (perhaps that reason is specifically, the beginning of the problems In the marital relationship in general between the two parties, because many of the injured men are reluctant to talk about the matter, and prefer to suffer in silence!)

Figures and statistics about prostate enlargement ..

  • Almost all men are prone to BPH as they get older.
  • More than 50% of men over the age of 60, and 90% of those over 80 years of age, have BPH. However, urinary symptoms only appear in 1 in 2 infected men.

People at higher risk of BPH / risk factors.

  • Aging and getting old.
  • Family history of the disease
  • Unhealthy lifestyle and lack of exercise and movement
  • Obesity
  • diabetic
  • Various heart diseases
This prostate enlargement is not a precancerous condition and it does not increase the risk of prostate cancer.

Potential complications of benign prostatic hyperplasia

Men with BPH are more likely than others to have the following problems:

1- Urinary tract infections: This is due to compression of the urethra, which leads to retention of part of the urine almost continuously. Inflammation is due to the fact that the urine contains a certain amount of bacteria and sepsis, and these naturally remain in the bladder for periods as a result of that compression, which ultimately leads to the occurrence of these infections. If urinary tract infections recur, surgery may be considered to reduce the size of the prostate and open the urinary tract.

2- Acute urinary retention in the bladder: When the urethra is completely compressed, urination becomes impossible, causing severe pain. This is a medical emergency. A catheter is inserted into the urethra to empty the bladder.

3- Bladder stones: Mineral deposits can form and cause inflammation and irritation of the bladder wall and prevent urine flow.

4- Bladder walls dilate: BPH can accelerate bladder wall aging. Over time, it loses elasticity and its contractions are less effective. Sometimes the bladder regains tension after prostate reduction surgery.

5- Kidney damage: Chronic retention of a certain amount of urine in the bladder and frequent urinary tract infections can damage kidney function in the long run.

Can hemorrhoids be treated in Turkey?

When should a doctor be consulted?

prostate enlargement : It is important to see a doctor in the event of symptoms of an enlarged prostate, especially problems with urination, because early detection of the disease facilitates treatment and reduces the risk of serious complications. In many cases, no symptoms may appear despite an enlarged prostate.

Which tests are done?

With a digital rectal exam, a doctor can detect an enlarged prostate and track its progress. This test is also used to detect the presence of prostate nodules and to assess the risk of developing cancer.

A urine and blood test to measure PSA levels may be performed as appropriate.

Treatment of benign prostatic hyperplasia

Treatment for BPH varies according to the size of the enlargement, the stage of the disease, the age, and the general health of the patient.

There are medications, then minimally invasive treatments, and the last option if all of the above don’t work, and that is surgery. The largest proportion of people with BPH go to the surgical option.

Treatment of benign prostatic hyperplasia at our center in Turkey.

1- Transurethral resection of the prostate (TURP)

The surgeon inserts an endoscope into the urethra to remove the prostate while preserving its external part.
Transurethral resection of the prostate (TURP) generally relieves symptoms quickly and provides a stronger flow of urine soon after treatment.
Temporarily, you may need a catheter to drain the bladder after TURP.

2- Transurethral incision of the prostate (TUIP) prostate enlargement

This time the surgeon makes one or two small cuts in the prostate gland. Which gives room for the passage of urine through the urethra.
This type of surgery is the best option for you if your prostate gland is small or moderately enlarged, or if you have health problems that make other surgeries risky.

3- Transurethral Heat Wave Therapy (TUMT)

The inner part of an enlarged prostate gland is destroyed by microwaveable thermal energy, causing it to shrink and facilitate urine flow.
Transurethral heat wave therapy may help partially relieve symptoms, and it may take some time before you notice results.
You may need to repeat the command after a certain period of time, if you choose to treat with this technique.

4- Transurethral needle ablation (TUNA)

Here a needle is inserted into the prostate gland.
Radio waves pass through the needle, burning and destroying excess prostate tissue that’s blocking the urinary tract.

5- Laser treatment of enlarged prostate

The high-energy laser beam destroys or removes enlarged prostate tissue.
Laser therapy generally relieves symptoms immediately and has a lower risk of side effects than non-laser surgery.
Laser therapy can be used on men who need other prostate procedures due to their health condition.

questions and answers

1 ~ What tests should be performed to diagnose BPH?

After your doctor has reviewed your medical history, he will conduct a clinical examination that examines the digital rectum of the prostate. The doctor uses a finger to assess the size, shape, and consistency of the prostate. Your urinary symptoms will be carefully evaluated, blood and urine tests performed, and urine flow velocity measured during flow cytometry.

2 ~ For the symptoms caused by BPH, why would a doctor order an FSA test?

Because it can know the activity of enlarged prostate and the probability of prostate cancer.

3 ~ What is the purpose of urine examination?

A urinalysis is used to rule out a urinary tract infection that causes similar urinary symptoms. If you have an infection, a urine culture is used to identify the germs and choose the most appropriate antibiotic treatment to treat the infection.

4 ~ What is the most common surgery for BPH?

The recommended surgical treatment for symptoms of BPH is TOR. The goal is to remove the part of the prostate that is causing the symptoms (adenoma). The operation is done through the urethra without making an incision in the skin. This technique is considered a minimally invasive treatment. It provides an optimal improvement in symptoms caused by symptomatic BPH.

5 ~ Is there a relationship between surgical treatment and erectile dysfunction?

In most cases there is no connection. But it is the damage to the nerves surrounding the prostate during its removal, especially with classic surgery, that causes erectile dysfunction.
